Dustyn Lee Adam, 30, of Fairfield, died Aug. 11, 2012, at his home.
According to his wishes, the body was cremated.
A private graveside memorial service will be at a later date.
Visitation will be from 6-8 p.m. Thursday at Behner Funeral Home.
