OTTUMWA —
Dortha LaVelle Cook, 96, formerly of 248 S. Davis St., died at Pleasantview Home in Kalona on Friday, Dec. 14, 2012.
She was born Jan. 20, 1916, in Fairfield to William and Anna Hannah Reynolds.
Dortha married Therion E. Cook on Dec. 31, 1938 at their home in Ottumwa.
She graduated from Fairfield High School and Iowa Success School in Ottumwa.
She was a member of the First Church of God in Ottumwa and attended First United Methodist Church in North Liberty. She was also a homemaker.
Dortha lived with her son and daughter-in law, Gordon and Marilynn Cook, from 2000 to 2010, and the past two years at Pleasantview Home in Kalona.
Dortha is survived by six sons, Royce (Virginia) Cook, of Overland Park, Kan., Dr. Gordon (Marilynn) Cook, of North Liberty, Roger (Dorothy) Cook, of Corona, Calif., Melvin Cook, of Salem, Ore., Gaylen Cook, of Iowa City, and Dr. Michael (Elaine) Cook, of Bremerton, Wash.; 10 grandchildren; and 11 great-grandchildren.
She was preceded in death by her husband, parents, a sister, Lola Axtell, and three brothers, Jesse Reynolds, Wray Reynolds and Curtis Reynolds.
As were her wishes her body has been cremated.
A memorial service will be held at 11 a.m. Jan. 5 at the Church of God in Ottumwa; officiated by Rev. Stanley Reilly.
In lieu of flowers, memorials may be made to Pleasantview Home and Iowa City Hospice.
